Social Worker - Transition Team - Surrey

Are you an experienced Adults Social Worker looking for a new role? Surrey County Council are looking for a Social Worker to join its dedicated Transition Team in Woking/Reigate.

Duties:

• To provide a high standard of social work to users arising out of the Council's duties.
• To facilitate a smooth transition for young people with a disability and/or learning disability and autism from into adults services.
• Within the context of an enablement approach and operating within an integrated service, to provide individualised, outcomes-led, personalised support, based on structured, individual assessments that inform the setting up and commissioning of appropriate enablement packages of support.
• Undertake the more complex assessments, packages of care and long-term support for people who are eligible for services that require a professional/ specialist input.
• To promote wellbeing and independence.
• To enable individuals and carers to maintain their confidence.

Person Specification:

• SWE Registration
• Eligible to work in the UK
• Drive and have access to a car
• Post-qualified experience in Social Work Teams
• Degree level or equivalent in Social Work
