Social Worker - Hospital Team

West Sussex County Council are seeking an experienced and compassionate Hospital Social Worker to join their dynamic team at St Richard's Hospital. This is a fantastic opportunity to make a real difference by supporting individuals during critical stages of their healthcare journey.

Working within a fast-paced hospital environment, you will play a key role in supporting safe and timely discharges, promoting independence, and ensuring individuals receive the care and support they need.

This is a hybrid position, with a requirement to be onsite 2-3 days per week at the hospital, alongside home working.

You'll be part of a multi-disciplinary team, working across:

• Hospital wards
• A&E and the emergency floor
• Community settings post-discharge

Key Responsibilities:

• Deliver a statutory social work service within hospital settings
• Undertake strength-based assessments and develop care plans
• Support discharge planning to maximise independence
• Attend ward rounds, family meetings, and MDT discussions
• Work collaboratively with partners across health, social care, and voluntary sectors
• Respond to referrals and complete assessments within required timescales
• Contribute to safeguarding enquiries where required

Requirements

• SWE Registration
• Eligible to work in the UK
• Full driving licence & access to a vehicle
• Post-qualified Social Work experience
• Degree in Social Work (or equivalent)
