Social Worker - Care Leavers
Connect2Socialwork are currently recruiting an experienced Children's Social Worker to join the Children in Care (CIC) Team within Halton Borough Council. This role specifically focuses on supporting young people aged 16 to 18 as they prepare to transition to independence or leaving care services.
Key Responsibilities:
- Manage a caseload of young people aged 16-18 within the Children in Care pathway.
- Undertake statutory visits, reviews, and care planning in line with legislation and local procedures.
- Work collaboratively with Personal Advisors, IROs, education providers, housing, health services, and other professionals to support the transition to independence.
- Ensure young people are prepared for leaving care by promoting their participation in pathway planning and decision-making.
Requirements as a Social Worker include:
- SWE Registration
- Eligible to work in the UK
- Drive and have access to a car
- Post-qualified experience
- Degree level or equivalent in Social Work
