Social Worker - Adults

We are seeking an experienced Qualified Social worker with the Adults Home First Team. Dedicated to facilitating efficient and effective hospital discharges. This is an excellent opportunity for practitioners with a passion for strength-based practice and experience in complex case management, including work related to the Court of Protection.

Key Responsibilities:

• Support hospital discharges by coordinating with healthcare providers and community resources.
• Conduct assessments and develop care plans that promote independence and well-being.
• Work collaboratively with individuals, their families, and multidisciplinary teams to ensure smooth transitions from hospital to home.
• Apply a strengths-based approach to support individuals in achieving their personal goals.
• Navigate complex cases, including those involving the Court of Protection, ensuring legal and ethical standards are met.
• Travel within the community to provide support and conduct assessments as needed

Requirements as a Social Worker include:

• SWE Registration
• Eligible to work in the UK
• Drive and have access to a car
• Post-qualified experience
• Degree level or equivalent in Social Work
