Adults Social Worker - Transitions Team

Connect2SocialWork are currently recruiting for an experienced Adults Social Worker to join a well-established Transitions Team in Halton.

This is a fantastic opportunity to support young people aged 14+ as they move from children's to adult services, ensuring smooth, person-centred transitions in line with Care Act eligibility criteria.

Key Responsibilities

  • Screen incoming referrals within the Transitions Team
  • Complete Care Act assessments in line with statutory requirements
  • Develop and implement person-centred support plans
  • Work collaboratively with young people, families, and multi-agency professionals
  • Support effective transition planning into adult services
  • Maintain accurate, timely, and compliant case records

Requirements

  • Qualified Social Worker with relevant professional registration
  • Strong experience completing Care Act assessments
  • Excellent assessment, analytical, and decision-making skills
  • Proven ability to work effectively with families and multi-disciplinary teams

Desirable

  • Experience working with Looked After Children (LAC)
  • Background in Transitions or Disability teams

Essential Knowledge

  • Strong understanding of how disability impacts family life
  • Ability to engage sensitively with young people and families during key transition stages
