Connect2Socialwork are currently recruiting for an experienced Qualified Social Worker to join the Short Break Service for Children with Disabilities Team within Halton Council.

Key Responsibilities:

• Manage a caseload of children and young people with disabilities in line with statutory requirements and departmental policies.
• Undertake assessments, care planning, and reviews to ensure individual needs are met effectively.
• Work in partnership with families, carers, education, health professionals, and other agencies to deliver coordinated support.
• Promote safeguarding and ensure all practice aligns with relevant legislation and guidance.
• Support children and families to access short break services that meet their needs and preferences.
