Senior Specialist CHC Assessor
Connect2Socialwork is currently seeking an experienced Senior Specialist CHC Assessor on behalf of a local authority based in North Kent. This is a pivotal role for a qualified nurse or social worker with a strong background in Continuing Healthcare (CHC) assessments and complex case management.
Role Overview:
The successful candidate will take the lead on Decision Support Tool (DST) assessments to determine eligibility for NHS Continuing Healthcare funding. They will manage a clinical caseload, including complex cases involving disputes and joint funding arrangements, while providing empathetic support to individuals and families throughout the assessment process.
Working collaboratively with internal teams, care providers, and local authority partners, the assessor will ensure appropriate care packages are in place-regardless of CHC eligibility. The role also includes conducting Section 117 reviews and contributing to service improvement initiatives.
Salary: £45,830 - £51,130
Key Responsibilities:
- Lead and coordinate CHC assessments and reviews (virtual and face-to-face)
- Manage a clinical caseload from assessment through to resolution
- Collaborate with stakeholders to arrange care packages and placements
- Provide specialist advice on CHC, Section 117, and joint funding pathways
- Maintain accurate records and support data reporting and governance
- Contribute to performance targets and service development
- Support training, induction, and continuous improvement efforts
Candidate Requirements:
- Registered with NMC (Nursing and Midwifery Council) or SWE (Social Work England)
- Significant post-qualification experience in nursing or social work
- Strong knowledge of CHC, Mental Capacity Act (MCA), and consent processes
- Excellent communication, report writing, and IT skills
- Ability to work independently and manage a dynamic caseload
- Full UK driving licence and access to transport
Desirable:
- Experience with Section 117 aftercare
- Background in mental health, learning disabilities, or older adult services
Working Style:
- Hybrid - a flexible working arrangement whereby an employee can undertake their work either in the office and/or partially or mainly from home on a temporary or permanent basis.
