Qualified Social Worker -Hospital & Community Discharge Teamm
Connect2Socialwork is recruiting on behalf a local authority in the South East of England for an experienced Senior Social Worker to join their Adult Social Care division. This pivotal role supports the delivery of a high-performing, person-centred service that promotes independence and wellbeing for the residents.
Key Responsibilities
- Lead and supervise a small team of social care professionals, ensuring high standards of practice through coaching, mentoring, and training.
- Manage a complex caseload, demonstrating expert practice in high-risk situations and safeguarding vulnerable adults.
- Provide professional advice and guidance to colleagues, contributing to service development and quality assurance.
- Support early intervention strategies through strength-based assessments and holistic care planning.
- Represent the council in multi-agency meetings, panels, and forums, ensuring collaborative working and effective resource use.
- Promote and implement the Council's values, behaviours, and strategic goals, including the Council Plan.
- Maintain up-to-date knowledge of legislation, policy, and best practice, ensuring compliance and consistency across the team.
- Lead on lone working procedures and support the OT Duty service.
- Contribute to continuous improvement through audits, performance monitoring, and feedback mechanisms.
Requirements
Essential Qualifications:
- Degree in Social Work
- Registration with Social Work England
Desirable Qualifications:
- Practice Educator Award, Best Interest Assessor, or Approved Mental Health Professional qualification
- Evidence of ongoing professional development
Experience:
- Minimum of 2-3 years post-qualification experience
- Proven ability to manage complex safeguarding cases
- Experience in supervision, performance management, and service development
- Skilled in chairing meetings, allocating cases, and supporting team members
- Familiarity with performance dashboards and KPIs
Skills & Knowledge:
- Strong understanding of relevant legislation, safeguarding frameworks, and social care policies
- Proficient in Microsoft Office and case management systems (e.g., MOSAIC)
- Excellent analytical, decision-making, and communication skills
- Ability to manage budgets and identify cost-saving opportunities
- Commitment to equality, diversity, and inclusion
Additional Information
- This post is exempt under the Rehabilitation of Offenders Act 1974 and requires an Enhanced DBS check.
- Applicants must hold a full UK driving licence (reasonable adjustments will be considered).
- The role includes responsibilities under the Civil Contingencies Act 2004 and may require participation in emergency response duties.
