Are you an experienced and passionate Hospital Social Worker looking for your next opportunity? We are supporting Hampshire in recruiting skilled Social Workers to join their well‑structured and highly supportive Hospital Team.

About the Role:

As part of the Hospital Discharge Team, you will:
  • Manage a caseload of patients requiring safe and timely discharge from hospital settings
  • Assess individual needs and develop, review, and implement effective discharge plans
  • Complete relevant assessments, documentation, and reports to support decision-making
  • Work closely with healthcare professionals, families, and multi-agency partners to coordinate care
  • Ensure individuals experience a smooth transition from hospital to home or appropriate care settings, promoting their safety, independence, and wellbeing

Requirements:

  • Qualified Social Worker (SWE registered)
  • Previous Hospital experience
  • Strong safeguarding knowledge
  • Ability to work effectively in a fast‑paced environment
Requirements as a Social Worker include:
  • SWE Registration
  • Eligible to work in the UK
  • Drive and have access to a car
  • Post-qualified experience
  • Degree level or equivalent in Social Work
