Hospital Discharge Social worker based in Hackney!

The job will involve the usual frontline tasks, including:

• Completing Care Act Assessments
• Conducting Care Act Reviews
• Performing Safeguarding and Human Rights Assessments
• Completing CHC Checklists
• Collaborating with hospitals, families, and other agencies

Requirements as a Social Worker include:

• SWE Registration
• Eligible to work in the UK
• Drive and have access to a car
• Post-qualified experience
• Degree level or equivalent in Social Work
